Student vaccine-verification and -exemption form

Encourage all students to submit their information as soon as possible.

As this information is critical to Metropolitan State University of Denver’s ability to plan for the fall semester, faculty and staff members are asked to support this effort by encouraging all students to submit their information as soon as possible.

Emails contain a unique, personalized link for the brief vaccine-verification or -exemption form, which does not require citizenship or health-insurance information.

Should students still need to get vaccinated, the Health Center at Auraria continues to distribute vaccines at the 5th Street Garage (650 Walnut St., Denver, CO 80204). The drive-thru site offers vaccines free of charge to all members of the community ages 12 and older. Students do not have to have proof of citizenship or health insurance to receive the vaccine. Scheduling an appointment is preferred, but walk-ups will be accommodated.

Vaccine-clinic hours:

  • Monday – 8 a.m.-5 p.m.
  • Tuesday – 1-8 p.m.
  • Wednesday – noon-5 p.m.
  • Thursday – 8 a.m.-5 p.m.
  • Friday – 8 a.m.-5 pm.
  • Saturday – 8 a.m.-5 p.m. (through July)

If a student is not in the Denver area, they can use Vaccines.gov to find a vaccine near them.
