Online student immunization form is now available

Forms for Health Center Auraria are available, too.

The Health Center at Auraria has taken student immunization paperwork online. The center and the Auraria Immunization Office teamed up to build an online version of the Auraria immunization form at healthcenter1.com.

“This new functionality is designed to make student compliance experience easier and reduce the verification processing time,” said Stephen Monaco, director of the Health Center Auraria.

Health center officials also said a Spanish version of the immunization form will also be available online by the end of the spring semester.

To find the form, visit healthcenter1.com, scroll down the left column to “Online Forms” and select the “Immunization Compliance Form.” Students can fill out the required form online, attach any required documentation and submit it directly from the webpage.

In addition, the Online Form section also includes a variety of other forms for students, faculty or staff who seek medical care at the Health Center. Each of these forms can be submitted through the website.

Health Center staff will inform patients how to fill out the forms prior to appointments. For same-day appointments, the forms can be completed at the Health Center.
